INSTRUCTIONS FOR IN-PERSON COUNSELING

Download, have faxed, or stop by and pick up the application and fill it out.

• Call CCCS (541) 926-5843 and make a appointment.

• Bring to your appointment

  • Your completed worksheet.
  • A list of questions you may have about CCCS
  • One month of pay stubs
  • All your recent statements (contact your creditors if you are not receiving statements to get complete account numbers, balances, and payment addresses)
  • All your credit cards.

INSTRUCTIONS FOR COUNSELING BY MAIL

Mail counseling applies to people intending to repay their debts. We do not conduct bankruptcy counseling by mail or internet.

• Print and complete an Application, the Statement of Counseling Service, the fee schedule and our Privacy Policy. Please be as complete as possible including all expenses and all sources of income (including any assistance you may receive from Social Security, the State of Oregon, child support, or a second job). Sign and return all four documents to our office.

• Write a brief note explaining your situation. How did you hear about CCCS? What would you like help with? What has caused your situation (reduced income, increased living expenses, medical problems, overspending, etc.)?

• List  any questions you have about our service.
